Virtual Care Centre (VCC) 

Operating Hours: Monday - Friday*: 9am - 5.30pm
*Excludes Public Holidays

A medical helpline for GPs to consult on all medical specialties and Cardiology. 
The helpline can provide advice on patients' medical conditions especially if the GP is considering sending the patient to the Emergency Department of the hospitals.

Based on residents’ health and social needs, NUHS CareHub helps to connect them with NUHS services and community partners. These include: 

  • Referral of seniors for physical/social activities at Active Ageing Centres (AACs)
  • Referral to the NUHS Community Care Team (CCT)
  • Referral for financial and/or family support at Social Service Offices (SSOs) and/or Family Service Centres (FSCs)

Support Groups & Services
Specialist Support Groups

Specialist support groups are messaging platforms where GPs can enquire about their resident’s condition and/or test results, as well as seek advice from NUHS specialists during office hours. There are currently two Specialist Support Groups for Cardiology and Orthopaedics.

NUHS Diagnostics supports polyclinics, General Practitioners, community hospitals, nursing homes, home care providers and the community in Singapore. CHAS Referral Form

The CHAS referral form may be used for:

  1. CHAS Medical Referrals to Subsidised Specialist Outpatient Clinics (SOCs) at Public Healthcare Institutions;
  2. CHAS for post-screening follow-up for abnormal Pap/HPV screening test result under Screen For Life (SFL);
  3. Referrals for post-CDS/Vaccination follow-up under the Vaccination and Childhood Developmental Screening Scheme (VCDSS); and
  4. Referrals for Breast Cancer Screening and GP-to-GP Referral for Cervical Cancer Screening under SFL and Healthier SG (HSG)
