The National University Health System (NUHS) serves as the Regional Health Manager for the western region of Singapore, caring for 1.14 million residents. Our role goes beyond providing medical treatments, we bring together hospitals, polyclinics, general practitioners, and community partners to build an integrated ecosystem of care.
Population health is at the core of this mission. We recognise that health is shaped not only by medical care but also by social support, environment, and lifestyle. That is why we focus on preventing illness, closing care gaps, and empowering residents to take charge of their health at every stage of life.
Through the NUHS Care Plan, we work with partners to prevent disease through education, anchor care in the community to reduce avoidable hospital visits, and deliver seamless, person-centred experiences across settings. This requires a whole-of-NUHS and whole-of-community effort, uniting expertise, and resources to build a healthier and more resilient population.
Our approach also emphasises smooth transitions of care. From discharge planning to follow-up in the community, we work with providers to ensure residents receive the right support at the right time. This coordinated system strengthens preventive health, improves outcomes, and ensures sustainability for future generations.
