Everyone’s health journey is unique, and no matter what stage of the health journey you are at, you will be able to find support and resources tailored to your needs.

Undergoing regular health check-ups and screenings, getting vaccinations, as well as understanding your health condition and making healthy lifestyle choices, are important in keeping you healthy. Together with our community partners, NUHS offers a suite of initiatives and events for you to embark on healthy living:

Getting Healthy & Living Well

We work closely with community partners to help you transition smoothly from hospital to community care after your discharge, so that you can receive appropriate care at home or closer to home.   

Our transitional care programmes such as the Hospital to Home Programme administered by the Community Care Team and the Outpatient to Community Programme ensure seamless transfer and continued care for patients to prevent medical complications and hospital readmissions.  

In addition to our network of polyclinics, we work closely with General Practitioners (GPs) to ensure that you have access to holistic and affordable healthcare. For those with chronic conditions, NUHS has several initiatives such as the Chronic Disease Self-Management Programme and Health Peers Programme to help you better manage your condition.
