We work closely with community partners to help you transition smoothly from hospital to community care after your discharge, so that you can receive appropriate care at home or closer to home.
Our transitional care programmes such as the Hospital to Home Programme administered by the Community Care Team and the Outpatient to Community Programme ensure seamless transfer and continued care for patients to prevent medical complications and hospital readmissions.
In addition to our network of polyclinics, we work closely with General Practitioners (GPs) to ensure that you have access to holistic and affordable healthcare. For those with chronic conditions, NUHS has several initiatives such as the Chronic Disease Self-Management Programme and Health Peers Programme to help you better manage your condition.
