Outpatient to Community

If you are visiting a Specialist Outpatient Clinic (SOC), the Outpatient to Community programme will benefit you.

Through close collaborations between our SOCs and primary care partners in the community, the programme facilitates the seamless transition of patients, who no longer require specialist care, to the community and be cared for closer to home.

For patients with multiple SOC appointments, the programme aims to deliver consolidated care helmed by one principal physician, either a specialist from the hospital or a GP.

The physician will also be supported by a multi-disciplinary team of nurses, allied health professionals and care managers. This model reduces multiple hospital visits and focus on treating the patient holistically with inputs from other specialists as needed.
