NUHS aims to partner primary care stakeholders and community partners to co-create a care eco-system of health and social services to support Healthier SG and social prescribing.
To foster stronger partnerships between NUHS, polyclinics, GPs and hospitals, we provide more holistic and coordinated primary care services, as well as opportunities for partnership and dialogue. In addition, continuous learning resources and relevant research are made available to GPs and clinic assistants.
