The NUHS Primary Care Network (PCN) is a network of General Practitioner (GP) clinics supported by NUHS nurses and care coordinators to provide team-based care in the community. If you are coping with chronic medical conditions, such as diabetes and high blood pressure, you could see an NUHS PCN GP in your neighbourhood instead of going to the hospital or polyclinic.
The true core of good medicine is the relationship between doctor and patient. It is important for a GP to have an honest and personalised conversation with the patient on his health and care management. In that way the patient will have full confidence in his GP's treatment and advices.

As a patient of the NUHS PCN, you can be assured that your healthcare is in good hands. Our PCN GPs work closely with NUHS nurses, care coordinators and specialists to seamlessly provide services you need to manage your long-term medical condition. You will have access to the following services:

As a patient of the NUHS PCN, you will be enrolled onto a chronic disease registry. Through this registry, we work closely with our PCN GPs to monitor your health and treatment outcomes to better manage your chronic condition.
