What are Community Health Posts?
Community Health Posts (CHPs) are touch points within your neighbourhood where you can get easy access to health services near your home.
At the CHPs, the community care team from NUHS provides residents with health counselling and education. If you have chronic diseases or conditions such as diabetes, high blood pressure and high cholesterol, we can help you in making better lifestyle choices so you can live better and stay healthy in the community.
The community care team, comprising community nurses and care coordinators, are sited in locations in your neighbourhood to bring care closer to you. We offer a range of services that include:
