My Health Map - Take Charge of Your Health

About My Health Map

My Health Map (MHM) is a preventive health management plan that empowers you to take the first steps to staying healthy. Developed by NUHS, MHM recommends health screening and vaccinations appropriate for individuals aged 40 years and above. These preventive health milestones are based on age and gender guidelines provided by the local health authorities.

Keeping track of your health status can help you make informed decisions about your lifestyle choices to improve your well-being and staying healthy as you age actively.

Chronic Disease Screening

Chronic disease screening is key to early detection of chronic diseases such as obesity, high blood pressure, diabetes and high cholesterol. Timely intervention can help prevent the development of such diseases, which could lead to serious complications that impact your quality of life.

Cancer Screening

Colorectal, breast and cervical cancers are some of the most common cancers in Singapore. Cancer, if left untreated can lead to death. The chance of recovery is a lot higher, when discovered early.

Vaccination

Pneumococcal vaccines help to prevent pneumococcal diseases, which can cause infection in the lungs, brain or blood. There are two types of pneumococcal vaccines, and when taken can provide long-term coverage. For individuals aged 65 and above, you are strongly recommended to take two doses of the vaccine for stronger immunity. Pneumococcal vaccination is Medisave claimable.

Influenza vaccine can help prevent influenza, which is highly contagious and can affect anybody. Usually recommended at a frequency of once a year, influenza vaccination is now Medisave claimable.

Functional Screening (Vision, Hearing & Oral Health)

Functional screening detects functional decline in vision, hearing and oral health among seniors in the community. Early detection and intervention improves seniors' quality of life, supports independence and enables ageing in place. The recommended screening frequency is once a year.
