Community Care Programmes

To enhance the delivery of care within the community, NUHS works in close collaboration with community health and social partners to better coordinate care and support in the heartlands. Through our community care programmes, we aim to help residents live and stay well by providing appropriate, convenient, holistic and value-added care for our patients.

Timely follow-up is vital for ensuring that you are moving forward with the correct treatment plan that drives positive care outcomes. Our community care programmes aim to bring community care services closer to home, ensure that you are receiving the medical intervention you require and equip you with the knowledge and resources to stay on top of your health. Together with healthcare providers and community care agencies, we are here to empower you to take ownership of your own health and co-create a healthy and engaged population of 1.12 million residents in the western region of Singapore.
