At NUHS, we are committed to supporting people with chronic conditions in the community. We aim to help residents live and stay well through our community care programmes

  1. NUHS Community Health Posts (CHPs)  
    Community Health Posts (CHPs) are touch points within your neighbourhood where you can get easy access to health services near your home. 

    At the CHPs, the NUHS community care team offers residents in the western part of Singapore health-related counselling and education. If you have chronic conditions like diabetes, high blood pressure, or high cholesterol, we can support you in making healthier lifestyle choices so you can live better and stay healthy in the community.

  2. Chronic Disease Self-Management Programme (CDSMP)  
    The Chronic Disease Self-Management Programme (CDSMP) is a six-week programme originally developed by Stanford University to help individuals manage their chronic conditions. This programme is designed for people living with one or more chronic conditions, as well as caregivers supporting them. 

    You will learn the following after enrolling in the programme:
    • Symptom(s) management
    • Nutrition and exercise
    • Communication skills
    • Relaxation techniques
    • Self-management and decision-making skill

  3. Health Peers Programme
  4. The Health Peers Programme is a three-month programme where volunteers are paired with you to set health goals and offer peer support in adopting a healthier lifestyle. The programme focuses on guiding you in healthy eating and regular exercise. Click here for more information on the programme.

Diabetes Support Groups

  1. Diabetes Singapore
    Diabetes Singapore is committed to raising awareness, providing education, and supporting individuals living with diabetes. The organisation facilitates a Diabetes Support Group and hosts public forums. Click on to learn more about Diabetes Singapore.

  2. TOUCH Diabetes Support
    Touch Diabetes Support engages in promoting diabetes awareness through various talks and events. To discover upcoming events, click here to contact Touch Diabetes Support.

  3. typeOne.sg 
    Based in Singapore, typeOne.sg serves as a support group for individuals with Type 1 Diabetes. This platform allows you to learn more about others’ experiences and share your own. Find out more about typeOne.sg.
