What do you need to enrol in the programme?

NUHS App* and WhatsApp installed on your mobile phone
A functional blood pressure monitor
You must be comfortable to receive and understand instructions in English or Chinese.

How do you navigate to the CHAMP enrolment form?

Appointment

Step 1:

In the NUHS App, click on the icon "Health Together".

Public Transportation

Step 2:

Select "Self-Help Tools".

Use a reusable bag
Step 3:
Select “CHAMP” to access the enrolment form.
