Personalised Care and Support Planning (PCSP)

Living with diabetes is a unique journey, and your care should reflect your individual needs, experiences, and priorities. Personalised Care and Support Planning (PCSP) – formerly known as PACE-D - is an approach designed to place you at the centre of your treatment decisions. 

With PCSP, your medical appointments are organised to enable you to take a more active role in managing your health. Your healthcare team will support and guide you in taking better care of your conditions independently.

How does it work?

  1. Once a year, before your annual review appointment with your doctor, you will receive a preparation letter, which includes your test results.
  2. Reviewing the letter allows you to think about your health, what matters to you, and any questions that you may want to ask.
  3. During your yearly review, you will discuss your letter and care choices with your doctor and care team.
  4. Together with your doctor, you will create a care plan that is personalised to your individual needs.

  • You will receive care that is tailored to your specific needs.
  • Your doctors and nurses will get to know you better, and you will foster a stronger relationship with them.
  • You learn more about your own health and become more confident in managing your conditions.

Where is PCSP available?

PCSP is currently offered at selected National University Polyclinics. To learn more, speak to your polyclinic doctor.
